You need to turn on "Rule#1" and "Rule#2" in your FW and web logs. That's
the only way you can tell what rule caused the lockout.

That response (12202) means that they've run afoul of some rule you have in
place on the ISA server. Without the log entry for the event, it's
difficult to say what rule caused it.

I have installed ISA Server 200 for several weeks. Some of my clients
often encounter the following error when trying to access some websites,
