It is most likely that you haven't set the ISA server to be in your IP routing stream (in a simple network, ISA needs to be your default gateway). This configuration is needed because server publishing (for inbound SMTP) uses SecureNAT, and for SecureNAT to work properly, the ISA server must be upstream in the IP routing path out to the internet.
