Custom Error for Banned Sites 12202

Hi,
I wish to configure a custom error page for Banned Sites 12202 on ISA
Server 2004.
Our ISA Server 2000 has a customised 12202.htm page which is presented to
users when attempting to access a banned site.
With ISA Server 2004, it seems to display the default error message with
only a reference to "The ISA Server denied the specified Uniform Resource
Locator (URL) 12202" rather than loading a separate 12202.htm page.
How can I configure ISA Server 2004 to display a custom message when users
attempt to visit a banned site?
