   Dear List
at the risk of being guillotined, I have a few loud thoughts - answers to
which may emerge slowly.
(1) what was the weather forecast when decision for the sortie was made?
(2) Did the pilots recheck the weather just before take off, if they had
some kind of weather warning?
(3) Did the DMetO find it prudent to inform the formation leader about
adverse weather or just the routine METARs were supplied to ATC?
(4)Did the COO and SFSIO actually monitor the prevailing flying conditions
and the flight safety environment?
(5) Did the Formation Leader undertake a proper briefing before the planned
sortie including the actions in case of an emergency?
(6) Did any communication undertake among the formation once in bad weather?
(7) Was the communication sufficient or did it rather distract the pilots of
the ill-fated aircraft from their recovery actions from the emergency?
(8) Did any lapse or omission of judgment take place on part of deceased
pilots?
(9) What was the likely workload on the deceased pilots - of actual mission
flying and once the emergency had set in? Did they avail of any other
resources - man or machine, outside of the aircraft?
(10) Were there any trait or characteristics amongst the crew, alive or
deceased, which led to the sad outcome?

I am thinking aloud because I have always felt that the communication, the
lack of it or sometimes the distraction that it causes - affects judgment
and decision making. And the end results invariably are not happy. Moreover
happy outcomes are not as well publicised to understand the dynamics of
positive communication leading to correct evaluation of situation and
decision making. Tell me if I am wrong?

I would also request the List instead of telling me not to question, to put
their own doubts forward. Invariably, the members take a logical approach in
an Inquiry, but mindgames occur later - by then the inquiry is final and one
can not alter, even if there are serious Human Factors related unanswered
questions...
