IBIS Veri6.0_wip2 is ready for review.

It contains corrections discussed at the August 9, 2013
teleconference and others found by several people.
The corrections include:
Adding Converter_Parameters to the Section 3.1 hierarchy diagram
Explicitly giving several Defaults in Tables 21, 27 and 31.
Several editorial corrections and table linking corrections.
Thanks to Mike LaBonte for most of these changes and
assembling the files.
More comments are welcome to me, Mike or the
editorial committee.
