Thanks. Also, going through the code further, I did some changes in
_InsertEndpointForPipe. The SetDequeuePointer was failing with a context
error, as a result of which, the queued requests for the endpoint were
never getting executed. Also, I added HUB code, to detect devices on non
root hubs. Didn't get time to make a patch. Will do it after a little more
testing, soon.
