this seems to be a hardware issue on my side. I think the laptop
overheated (hard drive and cpu/video chips) and is now unreliable. My
Ubuntu partition died on it while I was using Ubuntu. I have retired
r41142 CD to install onto another PC I have here and it's working just
fine. So closing this one out as invalid.
