Just a heads up: My solution to finding previously added entries by the
BPackageEntry pointer is wrong. Apparently, BPackageEntry instances are
re-used during visiting entries, so these pointers become stale. In the
outline, this will result in entries being added to the first list item at
the same level as the actual parent. I will try to think of something
different, but it is getting late...
