My mother didn’t actually use any rehab facility other than Hebrew Rehab, but
here are two tools that can help you search by location and rating. At least
it’s a place to start:

Folks had recommended Neville Center at Fresh Pond which scores very well, but
I saw mixed reviews.
You might also try Yelp to see consumer reviews. I found that very helpful
when I was looking for a place for my mother.
