Ok, so you're not creating these programmatically :)
Typical behavior is to create additional proxy addresses on the objects so
that you can receive for those objects and forward it to the proper
destination.  The RUS handles this and other items for you (there's a kb
about the exact pieces if you want to look).  If you don't create those
attribute values, then you'll need to let the RUS do it for you.  This
indicates that you'll have to remove that checkbox after the RUS has run.
Maybe nightly would be frequent enough?

So the logic would be something like:
Once a night, wake up and enumerate the contacts in a specific OU (or the
entire domain if so needed)
For each mailenabled contact: 
Deselect the recipient update service

Let me clarify. If you remove the checkbox before the recipient policy
updates the contact, then not even the custom SMTP address stays in there.
Then there is no email address whatsoever and you can't send to that
contact.

contact?

No. The desired result is creating a contact that only has an outside
(outside of our organization) SMTP address, without the recipient policy
adding the additional username@xxxxxxxxxxxxxxxx and
username@xxxxxxxxxxxxxxxx addresses.

If you uncheck that box before it populates, the SMTP address never shows
