By saying you have the PIX configured very well, does that mean you are using the SMTP fixup protocol? If so, you are going to have problems. The firewall should not be doing any kind of SMTP proxy unless is it is app aware, like ISA is for Exchange. You must have your Exchange server locked down for relay settings else some spammer will find it and use it as an open relay.
