[etni] oral exam for bagrut

Could someone please explain to me how to apply for an oral bagrut (5 points)
examination for my students. I imagine it involves filling out a form and
faxing it to someone, but I can't find the form on the English Mafmar website.
My school is in Beit Shemesh, if that makes a difference.
