Day 27 Doctor: Bob Piecuch Nurses: Leah (night), Ruth (day) RT: Amy (day) Gestational age: 32 5/7 weeks Weight: 1564 g Length: 40 cm (5/5/14) FiO2: 25% Flow: 1.5 LPM on NC Feeding: 30 ml Q 3 hr. (24 cal breast milk with 1 ml liquid protein added.) through NG tube. PO multivitamin and caffeine. New orders: increased feeding regimen, chest X-ray tomorrow morning Other issues: left lung ongoing PIE Other notes: ongoing weaning of oxygen support .5L q 12hr as tolerated, little to no residuals, elimination good Today was mostly more of the same for Enzo, and that's great! Enzo had a peaceful evening last night. When Leah weighed him, he tipped the scale at almost 30 grams heavier. That means he's put on 258 g in the last week! Adam spent the evening working on finding the right balance for his respiratory support. Enzo did great at 1.5 LPM and 25%. They tried him at 1 LPM, and he lasted about 20 minutes before he started losing blood-oxygen saturation. Amy monitored him closely throughout he day and at the 2 o'clock feeding, she was comfortable changing his hardware out for a standard nasal cannula. Enzo misses his "handlebars", but he's so much more comfortable now! Only thing is that he has to find something else to grab onto.
Our little guy was unusually fussy today (but never inconsolable). I can only assume that he's still recovering from yesterday's trauma, even though the only physical trace was a little extra goop in his eyes. It just seemed like he could never find a comfy position to lie in. He's finally moving up in the feedings (increasing the quantity per body weight)--today's ration was an ounce of fortified breast milk plus 1ml of liquid protein each meal. Even though he's ramping up quickly, he has had very little "residual" food left in his tummy when the next mealtime rolls around, three hours later. We're looking forward to even more progress as he continues tolerating the increased quantity. Also, he's grown so much that the NG tube was moved from 16 to just shy of 18! After all, he's now more than 25% heavier than he was at birth! Dr. Bob, who's director of the NICU, ordered a follow-up chest X-ray for tomorrow morning. (They're usually done at about 5 a.m.) This will be Enzo's first since 4/28, and it will be interesting to see the change that's happened.
