I haven't seen an intake form yet but our group uses a form for case closing
data which I posted on the listserve database. If you go to the Collablaw home
page on Yahoo, you will see a menu on the left side of the screen. Click open
"files" and scroll down to the 'NCLG Case Report' file. There are some other
files in some of the other folders that may also be helpful. If you don't mind
sharing your intake form, would you mind uploading it to the same database. And
while you are at it, try registering your contact info on the members database.
It's a useful shortcut to getting in touch with listserve members off list.

Our newly-formed group is starting to work seriously on targeted
educational and marketing efforts to make other professionals and the
public aware of collaborative law as a desirable alternative and to
identify those trained attorneys available to handle such cases. In
order to gauge the effectiveness of any one effort, we thought it
would be helpful to have all attorneys in the group complete and
submit a case report or intake form for each case, specifically
identifying the source of the referral or how the clients learned
about the process or that attorney. Thinking ahead, we also
anticipate wanting to have case closing reports to enable us to
collect hard data on the number of cases, length of time from opening
until closure of the case, number of settlements, etc.
Do any other groups have such forms or reports that they would be
willing to share? I tried to search the listserve archives but my
search terms did not yield any prior discussion of this. So I
apologize if this is an old topic. Thanks.
