Hi All,
Does anyone have a data request and conditions of release form for external
individuals or organisations that they are willing to share? We're in the
process of reviewing our policy and procedure on the provision of data outside
of our organisation. The form should include details such as:
* Intended use of the data
* Storage, security and destruction of the data
* Ethics approval attached if relevant
* Disclaimer including compliance with relevant Victorian privacy
legislation
Your assistance would be greatly appreciated.
