Hello PCMH Peer Group members:
In preparation for our HCCN Program Summit on Thursday 07/31/2014, I want to
make sure that our HCCN PCMH Matrix is fully up to date in time for the Summit.
Please review the attached last version of the Matrix (07/10/2014), and, if
your health center has any updates that need to be made to the Matrix, please
send them over to me by EOB tomorrow (07/29/2014).
