I think the main elements are the book must have:
Title
Author (if there is no author that is usually ok, you just use the
publisher)
Publisher
Copyright holder
Copyright date
I think there has been mention in the past you have to have a title page,
but the cover of the book may be the title page, depending upon the type of
book and publisher.
