I'd be interested if anyone does have a solution. The workaround is to use chapter numbering 1-1, 2-1 etc and then you can = use RD fields to pull the TOC and the index together. It also makes easier to publish as you do not have to wait for the = preceding chapter to be finished before you print out the chapters.
