I've finally been forced to create a template that does not use the MS Word
abominable Outline Numbering system. Leigh Webber has a very good guide
(Multi-level Numbering using Fields) that I have used to help ease me
through the process. 
My template is almost complete. The numbering works fine, it's rock solid,
and it allows for multiple chapters that may also have more than one annex. 
My problem is that I cannot get a satisfactory solution to show the correct
page number in a TOC where an Illustration or Table exists in an Annex. I am
using a TOC field like this:
{TOC \t "Table_Caption" \s h1}  to base the TOC on the caption style that
includes the page number and chapter number ("h1" is the SEQ field for
chapter number).
This gives a page reference of, for example, 5-2 (for page 2 in chapter 5)
in the TOC.
What I want is to allow a TOC to show  page reference of, for example, 5A-2
(for page 2 in Annex A of Chapter 5). Also note that "a1" is the SEQ field
for the Annex number.

I'm hoping that there is a simple solution. Please help.

Yuck.

I would make the Annexures annex the book, not the chapter. This way you can
have A-2 as your page number. As your scenario stands, you cannot do it. If
you were to use Word's built-in numbering, you may be able to tweak
something more useful up, but you have stated this isn't so :-)

A pre-publishing macro could do it but. It would track through the paras,
collecting the latest SEQ results of the involved fields, and looking for a
para style that is Table Caption to make an entry in a table with.
