Try searching some of the major distributors. I was pleased to find a customer's old design the other day in the Dakota catalog. Do a search for some of the items in the design and see what comes up.

All to her downfall...shirts were cheap 100% lightweight, shrank and developed holes in one month at 3 times the price we quoted! Needless to say, he refused to buy another one-and wore that ratty thing every time the shirts were required to wave it in her face! But the were also other items in the catalog-all premade designs with a 'name drop' for the school/college/band etc. Your customer may have one of these-might have forgot where it came from or whatever. Unless they want to pay you to redigitize it-is it worth your effort to do it for nothing, embroider one shirt-and never see them again?
