Here's 3 things that have caused it for us over the past 4 years...
#1- behind the bobbin-I'm not a tech, but the spinning knife? or 
something...Tech took it apart, found nicks in it (from broken needle) and I 
used my talents from silversmithing to totally take them out and high polish 
it-fixed it.
#2-needle not at the right 'angle' twist in the needle bar. It's really 
fussy-not enough or too much and the thread rubs in the bobbin somewhere and 
'frays' out on you. Check your 'rotation' angle on the needles. Easy way-look 
at a needle that NEVER frays and match the rotation of that one.
#3-'Center Needle' alignment-your needle 'plate' isn't dead center-off just a 
half a hair-and the thread rubs against it and shreds. I might think that if 
ALL your threads are fuzzy-this is where to start, and take it off, use a 
magnifier and see if there are 'scratches' from bad needle in the hole!
