more often than I do that's for sure.

I would gues about once a month should be fine.

I don't think the order is absolutely crucial but I would guess it makes 
more sense to do a scan disk before a defrag, though if you get a serious 
number of errors it may be better to forget the defrag altogether and just 
prepare to get a new hard disk.
