VNC sends bitmaps down and it's vastly slower than ICA. I'm sure ICA must
send GDI commands, some of which will naturally be bitmaps.

I was wondering how smart ICA is, i.e. I guess it isn't simply sending bit
changes, but instead using some kind of x'like instruction set to at least
provide pattern repeating/windowing functions?

Your synopsis is about on the money.
ICA sends partial and full screen updates, X sends instructions (as a
very rough and ready comparison).
