It's also a good idea to wire up so at least one optic on each phase is
on a separate cable back to the controller from the rest - same
principal trying to create the conditions where at least one lamp is on
in fault conditions.

In Kirklees, and I other West Yorkshire Authorities we have been
following Mervyn Hallworth's 3rd unwritten rule about signal design
which is N = L+1

N = number of signal head heads and L = number of lanes. So for a 1 lane
approach this means a minimum of two signal heads. 

I prefer to have at least three heads for red lamp monitoring purposes.
If two reds are out the signals look like they are off on that
particular approach. With three heads there is still the comfort that
one lamp may still be showing a red, your Honour.

I believe the history of this is based around a court case regarding a
RTC 
where the judge stated that the lack of a primary head for the lane the 
driver was using meant that the driver could not be at fault in the RTC
as 
the driver could not see the nearside primary and the secondary due to 
vehicles obscuring the heads.

I would ask, what happened to caution and not proceeding unless it is
safe 
to do so? 

If you do have any information, can you provide details and any DfT 
comment.
If we have to start designing this way then this will obviously require 
extra islands on 3+ lane approaches  or gantries or more of those lovely

cantilevered masts !! yuk.
