I don't have the time at the moment to delve into all technicalities
and details of what you are trying to accomplish, however I just
wanted to mention that there is at least one commercial tool
available that is designed to accomplish the task you propose.
It is 'SPAR' written by Applied Simulation Technology (Apsim).
I believe Hspice has the ability to extract poles/zeroes
from a transfer function as well, but it doesn't generate
the rational function spice model for you.

Given a list of s-parameters SPAR extracts the poles and zeros and
genrates a spice compatible 'black-box' model which in the form
of a ratio of polynomials.

Beware though, that one must be careful that the generated approximation
(Pade' or whatever) exhibits both stability and passivity.
