Mark, what is happening is that your server is putting in a default e-mail 
address because you have not specified an e-mail address to have the e-mail 
sent from. 

You need to ensure that the e-mail address field on your form is called 
PHORM_FROM and that you use 

$PHORM_FROM = ph_DECLARE; in your config file. This will send the e-mail 
"from" that person. The autoresponder will also go to this person. 

If you are using multiple e-mails, you need to put: 

From: {{PHORM_FROM}} 

as a header to get the e-mail to be sent from that adress.
