GUID's

Hi all,
anyone know the logic behind GUID generation? we have been in situations
where we see duplicate (yup) guid being generated by the system, a SR is
open but no ETA. I have seen couple notes that this is well known to happen
on AIX (we use that too), So the question is,
1. How the guids are generated (i mean i understand it uses time stamp and
all but what other parameters are used) ?
2. what would cause it to generate dups ?
this is for 11gr2
