Deadlock inserting into same rowid (different block)

Object rowed 199909 is a FK on child table.   The foreign_key is indexed (one 
column to one column PK in parent)

What scenario might cause this deadlock?   I am googling around but not finding 
this scenario - most discussions in insert revolve around indexing FKs.

SQL for both sessions is:
Insert into child_table (cola,colb,...) values (:1,:2,...);
