404 means file not available at
OA_HTML/jsp/fnd
either OA_HTML is not mapped to file system correctly (This is done via
"Alias" in http directives) or
File is really not available correctly .
Check in error_log at $IAS_ORACLE_HOME/Apache/Apache/logs where Web Server
is expecting this file .
