Paul: The most common cause for me is when I've somehow managed to leave mail on the server but my mac "thinks" I've deleted it (or vice-versa). The way I fix it is to go in to my mail through the web, bypassing my mail program completely, and clear out older stuff that may be lingering in the inbox.
