I got similar issues and traced it in the end to the Spam filter in place, which was denying some emails, but not others, dependent on what words were in place, and then sending out a 'fake' NDR to suggest you couldn't send spam to me in the future. I see then randomly. If I resend the e-mail a few seconds later it will go through fine. My other users are seeing similar issues when sending to other domains.
