Just use voiceover commands to navigate to the specific attachment you want.
Interact with the text area of the message, go to the end of the email by
pressing VO shift end, and put the voiceover cursor on your attachment by using
VO shift left or right arrows. You should then be able to press VO space on it
to bring up the context menu.

Hi Everyone,
I’m almost embarrassed to admit this, but I’ve been using my Mac for
almost 8 years, and still have trouble opening attachments in an email
message when there are multiple attachments. I have an iMac Early 2009
running El Capitan. I use the Apple Mail program.
Right now I have an email with 3 attachments. I press command + downarrow
to go to the end of the email message and then highlight the attachment I
wish to open. I then press VO Space, and choose Open Attachment from the
context menu that pops up. The first attachment opens even if it appears
that I have the third attachment selected. I try over and over moving the
insertion point to the attachment I want to open, only to have the wrong
one open. This is so frustrating. What am I doing wrong? There’s got to be
an easier way!
Thanks for any ideas or suggestions.
