I've created a xslt to do this using the toc.ncx, but just found an ePub which is valid that has a different name for the ncx file, so might be best to work from the odf file. Although most ePub 2s don't use Heading levels but normal p with styles that look like a headings so although braille can be produced it won't be formatted well. To be honest when you open up the ePubs it's quite complicated and only working with the creators of the ePubs to get them to properly format and stick to standard definition for formatting etc. will produce good results. If any one wants to look at the php code I've cobbled together to process ePubs and produce braille via liblouis they are welcome. I'm going via xhtml from the ePubs to dtbook and then into liblouis. I'm in the midst of upgrading to the latest version file2brl from xml2brl on a windows xp server. Also you could look at the DAISY pipeline 2 which has ePub 3 to zedai(DAISY XML) and zedai to PEF using the liblouis engine. this using xproc and xslts, Java etc.
