Outlook: message summary issues, plus can't remember how to send.

I have just embarked on one of my occasional efforts to switch my e-mail
usage to Outlook 2003.  I am having two issues I don't understand.
 
First, I wish to have my messages grouped by subject thread so that I can
see all the messages with one subject as one item and decide whether or not
to open the thread, rather than as individual message summaries.  I've gone
into the message viewing options and "group by subject" is checked, but no
joy.  Is there some other arcane setting I have to turn on?
 
And, as I'm scrolling down through my message summary window, JFW is not
reading the entire summary.  I'm getting the from: but everything else is
being truncated.  If I subsequently up-arrow and down-arrow, the line is
read, and if I go into Jaws cursor, the text is there.  It seems to be
related to when the window scrolls, as I don't have a problem until the
highlight has reached the bottom of the window.  What's going on here, and
what can I do about it?
 
Finally, what is the blasted key command to send an e-mail.  I can find the
send button on the tool bar, but I can't find any menu item with a shortcut
key.
 
I am using JFW 7.1 and Outlook 2003.
