You can use a heading instead of a bookmark.  Select the text you want to 
have linked, go to insert, then hyperlink, press alt-a for 'place in same 
document', and select the heading from the list displayed in the 
dialog.  Tab to Okay.

although it isn't really hard to do this, it isn't always easy to have JFW 
tell you it was done.  If you move character by character, JFW will say 
'hyperlink' when you get to the link; if you are using sayall, though, it 
won't.  Turning on colors in the speech and sounds manager helps, because 
unvisited links are one color (blue, in our test document)
and visited links are a different color (we had purple).
