A few word questions

Hey guys,
I'm running version 10.2.
I have to create a form in word.
Now, I need to use the forms toolbar.
I need to know how to create a text box, and a combo box.
Anyone know how to do this?
We use them at work and reading them is not a problem, but I need to
create one of these forms myself.
Can this be done, anyone know how?
