I worked for 6 years with a young man in high school who was totally blind,
totally paralyzed (on a ventilator) from a brain tumor.  He used JAWS and
J-Say with much success.  He just needed someone to put on his
headphone/microphone, and press the button to start the computer, and we had
it set that JAWS and J-Say were running upon startup, so he knew what his
files were called to open them by voice.  Once he was set up, he could
dictate into his document, print, etc. with no problems.

He is still using it pretty much independently.
