The upstream proxy is resolvable

It works just fine but logs this error every now and again. Sometimes it
stops forwarding queries to the upstream server. It still returns queries
to sites that are specified as fetch direct. A restart of the firewall
service resolves the issue.

Does anybody know exactly what would cause a proxy chain loop error?
If the upstream proxy stops for some reason would it cause a proxy chain
loop?
