[Ilugc] brochures and application form designs using tex... how ?

Hi!

   Dhanabalan (our dcf admin) wanted to know if we could design good
application forms using tex.. Like those University application forms with
checkboxes and other stuff

CTAN doesn't seem to give me anything.
