No Mailbox being created when adding a user

I have this problem where when adding a user in Active directory and creating
an exchange alias/email for the user during mailbox setup the mailbox never
gets created in the exchange. This just started happening in exchange 2003 on
standard server, the RUS is also not updating the users email addresses and
not creating an x.400 stamp. dcdiag and netdiag show no errors.
the user is able to logonto the domain.
