Your Pre-registration forms for this Summers' 5th INTERNATIONAL CONFERENCE

In order for your Pre-Conference Registration to be processed, the "fully
completed form" must be received at the ETAI office BEFORE SUNDAY JUNE 6,
2004. Any registration forms that are incomplete or received afterwards will
not be accepted.
