Thanks. In my file, the first line has often the title (ascii), I assume
this is what's meant by first heading. Definitely not a cache issue, but it
does not work for me :-(

Use First Heading will replace the file name in the link with the first
heading found on the page.  If there are no headings it will still use
the file name - with underscores.
