Your changes will be retained and as E said the proofer will be grateful that 
you made them so he/she/won't have to, and the book will get into the 
collection that much quicker.

As far as I know, the changes will be retained. 
And, if there are smart quotes in the book, changing them to regular quotes is 
important because those smart quotes are not handled properly by the braille 
translator.

  I have just finished my scan of "Mayor of 
  MacDougal Street: A Memoir" by Dave Van Ronk with Elijah Wald.
   
  It was a beautiful scan, and the ranked 
  spelling is all done, and even yes, the index.
   
  I was thinking of trying for my first 
  time to follow the Tips in the manual where you do the replacements 
  etc
  Where you open the file in Word and do 
  the search and replace all of the various things like smart quotes and such 
  that I hear some of you have been talking about.
   
  Before I do it, though, I wanted to make 
  sure it is desired.  Or if really when I send up and upload it, and 
  submit the RTF file of the book.  Will those cnanges be 
  retained?
   
  I guess before I spend the time to do the 
  extra mile, I want to be sure that I am not doing it for no 
  reason.
