I should, perhaps, have added that the entry forms are on another page, linked to from the above. The direct URL for that is http://www.lta.org.uk/Tennis-Foundation/Tennis-for-Disabled-People/Visually-Impaired-Tennis/ and you have to go down to the "start competing" heading; it's under that.
