My point was, I wasn't there in the car and have no knowledge of why they ended up in that situation so I'd be reluctant to judge other people as being brainless when I stand without facts. We have some fairly recent object lessons in our industry - e.= g., Longford gas plant and Westralia) where people have been killed through= no fault of their own due to bad, nonexistent, or unread-unfollowed docume= ntation. We try to instigate a strong safety culture in our writers, documentation m= anagement and delivery systems, and equipment operators in order to anticip= ate dangers and prevent needless injury and death (and not just for legal r= easons). However, even a strong safety culture is powerless against thought= less fools. Irrespective of their friends and families, our species is probably better = off without such people - but all too often they take out others at the sam= e time they end their own lives through their lethally brainless actions or= inactions. Today I have also just finished reading details of the Staten I= sland Ferry accident in New York that killed 11 people because the pilot di= dn't report medical conditions and the ferry company didn't enforce or appa= rently even deliver their safety policy documents to their crews.
