I think I have the same issue. But I never realized it could be PADSynth
related. Restoring states of modified instrument patches sometimes fail
in my case, the instrument is loaded but no sound comes out of it. If I
manually reload the instrument patch it's ok again.

Thanks, that sounds highly feasible. From a first look, I can definitely
see a potential problem. The cure might be just a little harder to come by
though.
