There is a bug in Windows 9x that prevents sndvol from seeing more than one topology port in a single PNP device node. I raised this with Microsoft several years ago and the only solution seemed to be to create a bus driver for the card and then one child driver for each subdevice. I used the bus driver example in Walter Oney's book as the basis for this and eventually got it working.
