Ah the joys of extend.dat. It is stored in the local settings area which doesn't roam by default (if you are using roaming profiles). We have to do this for a non ts compliant app here and at each login, it copies over the extend.dat to the users local settings folder if they use that application. Not something I like doing as any changes they do to it get overwritten, however there isn't much we allow users to do anyway. I guess you could create one and copy it in for people. I believe it is just a cache of what is\was stored in the registry, so why not try deleting/moving it and see if that solves your problem and everything keeps working.
