It sounds to me like you did not log in again as admin in install mode after you applied an MS hotfix or SP. Add a batch file that does a change user /install into the admins startup folder called A.bat login once as admin and delete the file from the startup folder and put the machine back in change user /execute mode and see if it resolves it.
