It sounds to me like you did not log in again as admin in install mode after
you applied an MS hotfix or SP.
Add a batch file that does a change user /install into the admins startup
folder called A.bat login once as admin and delete the file from the
startup folder and put the machine back in change user /execute mode and see
if it resolves it.

The other thing to look at is Maxmpctmpxt settings. These settings will
affect login times.
See http://thethin.net/maxmpxct.cfm Search technet on the subject also.

Subject: [THIN] Logins hang when applying personal settings

Good morning all

This morning we had the Most Preferred and Preferred Data Collectors go
down in our farm of 16 servers. We had one other server lock up. Users
would attempt to connect and they would log in and get to the stage where
they were applying personal settings and it would hang.

We saw this a long long time ago but resolved it by moving the PDC Emulator
role to a server that did not server print queues as it appeared to hang
when mapping auto-created network printers.

Has anyone seen this before?

Cheers
