Paul,
The CC7 is sent out (all of the time, unless master channel is disabled) on the 
Master channel for the respective keyboard.  So the Swell Shoe will transmit on 
Channel 13; the Choir shoe will transmit on Channel 15, and when Great 
Unenclosed is selected, choir shoe data will also transmit on 12 (GT) and 14 
(Ped).
When any MIDI coupler is selected, then CC7 data will be sent on the respective 
MIDI coupler channel, unless the coupler is selected to send CC11, or fixed 
volume, or off.

control of MIDI messages that regulate volume (Control 7).

I suspect that it is the same as the general pistons are controlled on.
